Connecting Students with Support When They Need It Most

 

The SFCC Care Team is dedicated to helping students overcome challenges that may affect their well-being, academic success, or overall college experience. Made up of faculty and staff from across the college, the Care Team works together to connect students with campus and community resources that provide the support they need to succeed.

If you're concerned about a student, we encourage you to reach out. Early intervention can make a meaningful difference.

 

When Should You Contact the Care Team?

Anyone in the SFCC community, including students, employees, and visitors, is encouraged to report concerns about a student who may be experiencing difficulties or whose behavior has become concerning.

You may want to contact the Care Team if a student is:

  • Showing significant changes in behavior.
  • Experiencing a decline in attendance or academic performance.
  • Displaying signs of emotional or mental health concerns.
  • Struggling with personal hygiene or self-care.
  • Sleeping in class when this was not previously a concern.
  • Going through personal challenges that are affecting their ability to succeed.
  • Exhibiting behavior that is disruptive to others or raises concerns about their well-being.
  • Believed to be at risk of harming themselves or others.

You don't need to have all the answers or know exactly what's happening. If something doesn't seem right, it's okay to ask for help.

 

How to Make a Care Team Referral

If you're concerned about a student, you can submit an Incident Report to the SFCC Care Team or email careteam@sfccmo.edu.

Please note that the Care Team is not a 24-hour service, and Incident Reports are not monitored around the clock.

If you believe someone is in immediate danger or is experiencing a life-threatening emergency, call 911 immediately.

 

What Happens After a Report Is Submitted?

When a referral is received, the Care Team follows a collaborative process to determine the most appropriate response.

The process typically includes:

  1. The Care Team receives and reviews the Incident Report.
  2. Team members assess the information and determine the appropriate next steps.
  3. A Care Team member contacts the student to assess their needs and connect them with available resources and support services.
  4. If a student is believed to be at risk of harming themselves or others, appropriate college personnel and, when necessary, outside emergency responders will intervene.
  5. The Care Team works collaboratively with the student to develop a plan that helps reduce barriers to success and connects them with ongoing support.

Our goal is to help students address challenges early so they can continue making progress toward their educational goals.

 

Confidentiality

Information shared with the Care Team is handled with care and respect for student privacy. Reports are reviewed confidentially and are used only to determine an appropriate response and connect students with available resources.

Who can make a report to the Care Team? 

Faculty, Staff, Students, or members of the Campus Community 

 

What kind of issues does the Care Team handle? 
They intervene in situations involving severe mental health issues, signs of basic needs insecurity (food/housing), and concerning threats or aggressive behavior toward oneself or others. 

 

Is a submitted Care Team report confidential? 

Not entirely. While we will treat every situation with discretion and privacy, we cannot guarantee total anonymity if a situation becomes a life-safety issue. Reports are handled on a "need to know" basis. 
 
I submitted a Care Team report but haven’t seen any improvements.  What’s Up? 

Getting a student connected to campus resources takes time as does seeing visible results in a student who may be seeking additional support (i.e. Counseling Services).  Additionally, students must accept resources.  We understand that it is hard to watch a student struggle and as a result jeopardize their academics, but rest assured that the Care Team is doing everything they can to support the student.
