 Name Change/Address Change

A request to change your address may be made by filling out the Student Change of Contact Information form.  Please print legibly and allow a minimum of ONE WEEK for processing.  Return to the Registrar's Office at the main campus.

A request to change your name in the STAR system whether you are a student or an employee may be made by filling out this Name or Social Security Number Change form and submitting the required documents.  Please print legibly and allow a minimum of ONE WEEK for processing.

To change your name on your personal SFCC student records, you must present copies of your government issued photo ID, an official name change document (includes marriage or divorce reocrd, court records, military records or passport) AND your Social Security card reflecting the correct name to the Registrar's Office in Student Services in the Hopkins Building.

To change your name on your personnel and payroll records, present your Social Security Card that reflects your new name to the Human Resources/Payroll Office in the Hopkins Building.

To change your Social Security number you must provide your Social Security Card AND government issued photo ID.
