The following links provide access to the general admission application form, specific program admission forms, scholarship applications, residence hall application, and work study application.

The following applications are provided to you in Adobe Portable Document Format (PDF). If you do not have Acrobat Reader, you may download the free Acrobat Reader software in order to view these documents.

You will need to print, complete, and mail or fax the applications to the Student Services Office, located in the Hopkins Student Services Center on the Sedalia campus. The fax number is (660) 596-7472.

Admissions Application
Apply online or download a paper application. 
Required for ALL students.  There is no deadline; however, submitting your application six weeks before the semester you wish to begin is recommended. The application fee at SFCC is $25.

Practical Nursing (Year One Program) Application
Complete an Admissions Application and this application if you wish to apply for selection and admission into the Practical Nursing (year one) program.

Associate Degree Nursing - Advanced Placement (Year Two Program) Application
Complete an Admissions Application and this application if you wish to apply for selection and admission into the Associate Degree Nursing (year two) program. LPNs are eligible to apply for advanced placement.

Missouri Health Professions Consortium Occupational Therapy Assistant
Complete an Admissions Application and this application if you wish to apply for selection and admission into the Associate of Applied Science degree program for occupational therapy. All application materials must be submitted to the SFCC Admissions Office by June 30, 2010.

Radiologic Technology Application 
Complete an Admissions Application and this application if you wish to apply for selection and admission into the Radiologic Technology program. All application materials must be submitted by Feb. 1, 2010.

Dental Hygiene Application
Complete an Admissions Application and this application if you wish to apply for selection and admission into the Dental Hygiene program. Deadline for application for fall 2010 is January 15, 2010.

Use this application to apply for new need-based and merit-based scholarships.

SFCC Foundation Scholarship Application
Use this application to apply for scholarships available from the State Fair Community College Foundation

Housing contract and work study applications

Residence Hall Housing Contract
Use this link to print out a Housing Contract.  Please note that an Admissions Application must be on file or accompany a Residence Hall Contract in order for students to apply for housing space at SFCC.

Work Study Application
This application is required if you wish to work as a student employee in any campus position.
