The following applications are provided to you in Adobe Portable Document Format (PDF). If you do not have Acrobat Reader, you may download the free Acrobat Reader software in order to view these documents.
You will need to print, complete, and mail or fax the applications to the Student Services Office, located in the Hopkins Student Services Center on the Sedalia campus. The fax number is (660) 596-7472.

Allied Health
Associate Degree Nursing - Advanced Placement (Year Two Program) Application
Complete an Admissions Application and this program application if you wish to apply for selection and admission into the Associate Degree Nursing (year two) program. LPNs are eligible to apply for advanced placement.
Practical Nursing Program (Year One) Application
Complete an Admissions Application and this program application if you wish to apply for selection and admission into the Practical Nursing (year one) program.
Missouri Health Professions Consortium Occupational Therapy Assistant Application
Complete an Admissions Application and this program application if you wish to apply for selection and admission into the Associate of Applied Science degree program for occupational therapy. Students are required to submit all application materials to the Missouri Health Professions Consortium OTA Program office in Columbia by the deadline of May 31 for the cohort class that starts the following January.
Radiologic Technology Application
Complete an Admissions Application and this program application if you wish to apply for selection and admission into the Radiologic Technology program. Only this application packet will be accepted for the February 15, 2013 deadline. If you have previously downloaded an older application packet, discard it and complete this version for submission by the deadline. Dental Hygiene Application
Complete an Admissions Application and this program application if you wish to apply for selection and admission into the Dental Hygiene program. The application deadline for the program class starting August 2014 is February 4, 2014.
Diagnostic Medical Sonography
Complete an Admissions Application and this program application and eligibility form if you wish to apply for the diagnostic medical sonography program. These courses are part of the MoHealthWINs program funded by the U.S. Department of Labor and have eligibility requirements. This program is included in the curriculum for Allied Health and MoHealthWINs.

These courses are part of the MoHealthWINs program funded by the U.S. Department of Labor and have eligibility requirements. Priority application deadline for fall is 5 p.m. Oct. 3.
Phlebotomy Course
Complete an Admissions Application and this eligibility form if you wish to apply for the phlebotomy course. This course is part of the MoHealthWINs program funded by the U.S. Department of Labor and has eligibility requirements. Applications are accepted on an ongoing basis.
Certified Medication Technician Courses

Housing contract and work study applications
Residence Hall Housing Contract
Use this link to print out a Housing Contract. Please note that an Admissions Application must be on file or accompany a Residence Hall Contract in order for students to apply for housing space at SFCC.
Work-Study Application
This application is required if you wish to work as a student employee in any campus position.
